AQW 45458/22-27 – Support for stroke survivors and their families

Mr Patsy McGlone (Mid Ulster): To ask the Minister of Health to detail any steps he is taking to improve support for stroke survivors and their families during the transition from hospital to home.

Minister of Health: Supporting stroke survivors and their families during the transition from hospital to home is a priority within the Stroke Action Plan.

All Health and Social Care (HSC) Trusts have discharge planning arrangements in place following stroke, with multidisciplinary assessment and coordination of onward care. Where appropriate, patients are supported through strokespecific community stroke services to continue rehabilitation at home.

My Department and HSC Trusts have collaborated to develop enhanced information and support for patients and families at discharge, and use audit data to inform service improvements. Ongoing delivery remains subject to available resources.
